The treatment of tuberculosis (TB) is challenging in many low-resource settings, where it is generally ineffective, leading to treatment failures, drug resistance, and deaths. We aimed to assess the efficacy and safety of standard short-course TB regimens in adults in these settings. We systematically searched PubMed, Embase, Web of Science, the Cochrane Library, and Google Scholar for eligible studies published between Jan 1, 2000, and Jan 31, 2013, and registered the review on the PROSPERO International prospective register of systematic reviews (CRD42012002847). Eligible studies compared treatment of pulmonary and extrapulmonary TB with a standard short-course regimen in people with confirmed TB in a low- or middle-income setting. Eligible studies were in low- and middle-income countries; at least half of participants had human immunodeficiency virus (HIV) infection or were HIV-uninfected but had a positive tuberculin skin test; and TB diagnosis was confirmed by culture, microscopy, or a combination. We extracted data on numbers of participants enrolled, loss to follow-up, treatment success rates, treatment failures, relapse rates, and adverse events. Of 69 eligible studies, we included 43 randomised controlled trials, with 5 036 participants. Most (3894 [87%]) participants had pulmonary TB. Eighteen studies directly compared treatment of pulmonary TB with standard short-course therapy, and eight studies directly compared treatment of extrapulmonary TB with standard short-course therapy. Treatment of pulmonary TB with a standard short-course regimen had a pooled treatment success rate of 74·4% (95% CI 72·4-76·4) and a pooled relapse rate of 11·4% (8·2-16·0). Treatment of extrapulmonary TB with a standard short-course regimen had a pooled treatment success rate of 79·2% (72·6-85·1) and a pooled relapse rate of 7·9% (6·3-10·0).
